Step 1: Obtain and fill application in APEX lobby, return form to Room 251 between 8:30-5:30 p.m. Mon-Fri.
Step 2: Bring form verified by Athletics to Bursar (Shuster Hall 031) between 9-4:30 for payment.
Step 3: Member will be emailed instructions for ID card pickup in APEX 251 from 8:30-5:30 p.m. Mon-Fri.
